First Visit
Thank you for your interest in Izadi & Szeto & Associates, a dental practice dedicated exclusively to Microscopic & Surgical Endodontics. We are specialist members of the American Association of Endodontists. We look forward to being of service to you. Our expert team, led by respected endodontic specialists includes an experienced endodontic staff. We provide the highest standard of professional care in a friendly, comfortable environment. We hope that the information provided here answers many of your questions about endodontic treatment.

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, treatment can be done the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.
Please assist us by providing the following information at the time of your consultation:
- Your referral slip and any X-rays if applicable.
- A list of medications you are presently taking.
- If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) or require medication prior to dental cleanings (i.e antibiotics, for pre-med.)
X-Rays
If your previous dentist has taken recent x-rays (within 6-months), you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.

Financial Policy
We aspire to provide the finest care at the most reasonable cost to our patients. Because we are a specialty practice payment is due at the time services are rendered unless other arrangements are made in advance. We will work with you and your insurance company if applicable for the best method of your payment. For your convenience we accept Visa, MasterCard, Discover, American Express & CareCredit. You may contact us prior to your appointment with questions regarding your fees. Our experienced staff will work hard to answer any financial inquiries.
It is our goal to work with you from start to finish so that your total experience at Dr. Douglas Szeto & Dr. Reza Izadi & Associates is a good one.

Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.
We will send you a statement for balances not covered by your insurance. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated.
